MEDICAL : Palliative Care            
 

The Palliative Care Team is a group of professionals working together to enhance the care for patients living with cancer and other life threatening illnesses, their families and carers.

The Palliative Care Team have undertaken training in this field.  The knowledge and experience of the Team plays an important role in the care these patient receive and aims to help them improve their quality of life throughout their illness.

The Team is based at Bodfan, Eryri Hospital in Caernarfon.  They can be contacted Monday to Friday between 9.00 am and 5.00 pm on 01286 662775 or 662776 and at Ysbyty Gwynedd on 01248 384384, extension 5121.

The Palliative Care Team provides advice, support, nursing care, day care in some areas, help with practical problems of daily living at home, including provision of aids and equipment where necessary, help with mobility, gentle exercise and movement, treatment for chest conditions and massage.  Palliative Care is also provided in hospital.

The Team works with and support the Primary Health Care Team and other professionals involved in the care of the patient and their family.

Requests for the Palliative Care Team can be made through the:

  • General Practitioner
  • District Nurse
  • Hospital Consultant
  • Other Hospital Staff

or by

  • Phoning the Palliative Care Team directly (your GP and District Nurse will be notified)
