COMMUNITY HOSPITALS & REHABILITATION: District Nursing            
 

The District Nursing Service provides a broad range of nursing expertise within the home, private or local authority accommodation, health centres / clinics and GP Surgeries; primarily to all patients who are unable to attend / access GP Surgeries / Practice Nurses.

The principal aim of the District Nursing Service is to organise and deliver skilled evidence-based nursing care that is necessary to sustain patients and support carers in their own homes or in residential accommodation

District Nurse

District Nurse Teams are attached to General Practitioner Practices and are led by a District Nurse who is specially qualified to assess the patient's individual needs.  The team includes qualified nurses and trained Health Care Assistants.

Following referral, patients may expect:

  • A personalised assessment of needs and development of an action plan to meet those needs.
  • Skilled nursing care for people who are acutely ill, terminally or chronically ill, but who do not require hospital admission.
  • Teaching and supporting patients and their carers, empowering them to regain independence in everyday activities.
  • Advice on personal health and safety in the home.
  • Referral to other agencies with the individual patient's consent.

You do not need your GP's permission to speak with the District Nurse.
