Learning Disability Services are provided through “Community Support Teams” based in Arfon, Dwyfor, Meirionnydd and Anglesey which comprise of Community Nurses, Speech and Language Therapists, Psychologists and Physiotherapists. Each team works closely with colleagues in Social Service Departments and Primary Health Care Teams, ensuring that the health needs of people with a learning disability are considered and met consistently and effectively. The Challenging Behaviour Service is made up of two teams that work closely together and provides a service into Conwy as well as the North West Wales NHS Trust areas. The Additional Support Team (AST), is a community service, which works with adults and children who exhibit challenging behaviour. It works in addition to the staff in the Community Support Teams providing an extra level of support by employing the staff’s specialist skills and expertise with the client group. The AST works in conjunction with other statutory agencies including Education and Social Services. The Emergency Intervention Service (EIS) provides support to people with Learning Disabilities who experience crisis which threaten to remove them from their homes. Peripatetic support is provided in their homes to try and maintain their community placement and there is also a three-bedded house in Tregarth, which can be used for short stays when their community placement breaks down or threatens to do so. A wide range of other services can also be accessed through the EIS including Psychology, Psychiatry, Speech and Language Therapy, Physiotherapy and Community Nursing. A resettlement programme is currently in progress that will see residents from Bryn y Neuadd Hospital moving to small-scale accommodation throughout the North Wales area. During this process the individual needs of both residents and staff are being carefully considered. Part of the plan includes enhancements to Challenging Behaviour (Additional Support and Emergency Intervention) Services, and these will continue to be offered across the whole of North West Wales. Following completion of the Resettlement and Reprovision Plans, Bryn y Neuadd Hospital will continue to be the headquarters for the service, and some elements of service may continue to be provided on the site. These are an inpatient Assessment and Treatment Service and a small housing development for some people who have continuing health care needs. Currently the Bryn y Neuadd site also hosts an inpatient facility that provides a 'local' service for clients when the alternative is a placement out of North Wales. We have a duty and commitment to ensure that people with learning disabilities in North West Wales receive the highest quality of service, based upon individual assessed need. Clinical Governance provides the framework to develop and improve such services, provided wherever possible in ordinary settings, and delivered by a competent and committed workforce. |
