The Community Children's Nursing team is based at Bodwrdda in Caernarfon. They are a team of 4 paediatric trained nurses and they cover the whole of Gwynedd and Ynys Môn. They provide care and support for a number of children, young people and their families. Referrals come from a variety of places and consist of tertiary referrals from Alder Hey Children's Hospital in Liverpool and other hospitals in Birmingham, London and Manchester. The Children's Unit and Special Care Baby Unit at Ysbyty Gwynedd refer children who need extra care following discharge from the wards. Other referrals come from the Specialist Children's Services and if there is an acute or chronic illness which needs nursing care. A number of children that are looked after have complex health care needs and receive a package of care which is funded by the Local Health Boards. These packages vary from 10 hours to 100 hours per week. To honour this commitment to the child and his / her family, the Trust employs approximately 30 Health Care Assistants to deliver the service in the child's home or at school. There are also 4 trained nurses who provide respite to the most profoundly disabled children. The majority of the children with complex health care needs receive palliative care. The work carried out by the team also consists of clinical duties such as dressing changes, injections and checking plaster casts / splints. |
