Community Mental Health Teams

Community Mental Health Teams cater for clients aged 18 years and over who are suffering from mental health problems including psychosis, bipolar disorder and moderate to severe depressive episodes.

Delivery of service is based around multi-agency working through the formalised Care Programme Approach (CPA).  All clients are offered a formal assessment which seeks to identify their needs and assess risk, a range of specific therapeutic and supportive interventions on an individual or group basis.  Clients are offered cognitive behaviour therapy, anxiety management, assertion training and support groups for sufferers of depression.

The teams have a valuable role in the assessment of the needs of carers and families of clients.  Service users are invited to participate in decisions regarding their care and carer / user groups are represented on several planning and policy-making forums.

Community Mental Health Teams in Gwynedd and Anglesey are based on five sites:

The Teams work in close collaboration with a wide range of service providers in their areas.  Their close links with the Hergest Unit and with Support Bed Units ensure that clients can move easily between hospital and community services as the need arises.

Clients with long standing serious mental illnesses also receive a routine physical health check linked to the Local Health Board's Calon Lân Project.  This seeks to ensure that this high risk client group, who have a higher than normal risk of suffering physical problems, are screened regularly and where problems are identified, referral on at an early stage to prevent longer term physical problems.

The Arfon Community Mental Health Team is currently piloting an Integrated Care Pathway for Service Users with Psychosis

Community Treatment Service

The Community Treatment Service (CTS), which covers Dwyfor and Meirionnydd, provides qualified nursing and social work input linking up with Care Managers in providing extended services to clients with serious mental health problems.  It is able to offer intensive short-term interventions to this client group to enable them to be better managed when in "crisis" to negate the need for acute inpatient care.  It can also facilitate earlier discharge from the Hergest Unit and provide long-term therapy to seriously mental ill clients.

This is a predominantly home-based service, although clients can be seen in other settings.  They are currently based in Ala Road Health Unit, Pwhelli.

The service is flexible and covers weekdays, evenings, weekends and also provides a 24-hour telephone line to clients.
